Personal Health PCHD Fees Fees for services Effective July 1, 2006 Adult Health The Adult Health Program is committed to maintaining the health and improving the quality of life for people 18 years of age and older. Breast and Cervical Cancer Program We assist in finding these cancers early and provide screening to those at increased risk who are not financially able to pay. Family Planning The Family Planning Program provides education, social, and medical services. Child Service Coordination The North Carolina Child Service Coordination Program (CSC) offers a comprehensive, coordinated, interagency system of service for all families with children birth to five with special needs. Health Check / Health Choice Health Check and Health Choice can pay for all or part of a childs health care. Immunization Program The goals of the Immunization Program are to prevent or control vaccine preventable communicable diseases. Community Services Team The Community Services Team is one of the many projects Person County was awarded by a grant through the local Person County Partnership for Children. Maternal Health Program The purpose of the Maternal Health Program is to provide comprehensive maternity services to childbearing women who are unable to obtain these services through other sources due to financial limitations. Communicable Disease Program The purpose of the Communicable Disease Program is to prevent and control the spread of diseases. Maternal Outreach - Baby Love Offering outreach, advocacy and support services to Medicaid eligible pregnant teens and mothers with infants up to age one. Health Education - Health Promotion Equipping the community with information necessary to live healthy lifestyles. Laboratory The laboratory offers support to other programs in the health department with tests which are essential screening and diagnostic tools. Child Health The Person County Health Department's Child Health Clinic Program provides well child care and limited sick care to all children under 21 years of age who receive Medicaid, NC Health Choice, private insurance and self-pay based on a sliding fee scale. Lead Program Childhood lead poisoning is one of the most common pediatric health problems in the United States, even though it is entirely preventable. PostPartum - Newborn Home Visiting A Public Health Nurse is available to visit Medicaid eligible mothers and newborns in the home environment. School Health Program The purpose of the School Health Program is to assure that children achieve and maintain optimum health, in order to promote maximum physical, emotional and intellectual growth.
